Refund Policy
The Company does not under any circumstances refund for any services made through this
website or any of the Company’s third-party payment service providers, for any products or
services purchased in connection with the services provided by the Company. Although we do
our best to ensure that our customers and patients and/or clients are fully satisfied, any issues or
concerns related to the purchase of any product or services should be directed to: Marina D.

Scarsdale, NY 10583. However, under no circumstances shall the Company issue any refunds.
Any refunds that may be issued are solely at the discretion of the Company under a case-by-case
basis. This does not negate any portion of this no-refund policy.
Cancellation Policy
APPOINTMENTS AND CANCELLATIONS
The standard meeting time (“Session”) for physical therapy is 60 minutes. However, you may
request a longer Session time. Requests to increase the 60-minute Session need to be discussed
with the therapist in order for time to be scheduled in advance. Such a request is subject to an
increased fee. Please keep in mind, that if you are using insurance, they may not cover longer
sessions, or may require preauthorization. In the event that your insurance does not cover a
longer session, portion of the session, or the complete session, you are responsible to pay for the
full amount of the Session.
THE CANCELLATION POLICY IS NON-NEGOTIABLE. Any Sessions must be canceled or
rescheduled at least 24 hours before the Session. You will be responsible for the entire fee if
cancellation is less than 24 hours. There are no exceptions. You will be charged a late
cancellation fee of $75.00 if the cancellation is less than 24 hours. There are no exceptions. This
is necessary because a time commitment is made to you and is held exclusively for you. If you
are late for a session, you may lose some of that session time. If you are more than 15
minutes late for a session, you may not be seen and will be charged for the session. Not showing
up for a scheduled session will result in a full session fee which must be paid in full before
scheduling another session.
If you do not show up for your therapy appointment, your credit card on file will be charged.
Credit Card Authorization Policy
By purchasing a service or a booking through this website or any of our third-party payment
service providers, you authorize Marina D. Castellanos PT, PLLC to process your credit card
payment through our service provider, Square. We securely use Square to process all
transactions for services provided. You should read the terms, conditions, and other policies
contained on the Square website by clicking here https://squareup.com/us/en/legal/general/ua.
Upon check out, by clicking on “Agree” and submitting payment and/or saving your credit card
information to your patient profile, you waive any rights or claims against the Company for any
cybercriminal activity or hacking of your personal information and/or credit card information
including but not limited to fraud. By also proceeding with payment and the purchase of any
product or service on this website you hereby agree to the Cancellation Policy and Refund Policy.
Suggested text: When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.
An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
Media
